The first arthroscopies were carried out by Takagi in Tokio in 1918. In 1921 E. Bircher reported about his first arthroscopic experiences gained with a laparoscope.

MENISCUS INJURIES AND DISEASES

There are sickle-shaped cartilage disks or meniscuses between the shin bone plateau and the femural condyles. Their function is to adapt to the bone shape and provide guidance and stabilization of the knee joint. Mechanical loads cause the formation of tears or fraying. The most frequent injury patterns and lesions of the inner meniscus are the bucket-handle tear, radiar tear and parrot-beak tear.

ACL RECONSTRUCTION (ANTERIOR CRUCIATE LIGAMENT)

Reconstruction of the anterior cruciate ligament ACL using the hamstring has recently become of increasing interest. Fixation using endo buttons is
a common method. The disadvantages of this fixation away from the point of insertion are:

  • enlargement of the tunnel
  • creep at the tendon-tape transition along with giving of the tendon construct and permanent elongation (Höher et al*)(3.8+/-0.8mm). This is also known as the Bungee effect.

Fixation close to the insertion has been suggested and such methods have already been used successfully:

  • Fixation using an interference screw
  • Suspension of loops using transverse rods

Disadvantages of these techniques are:

  • hardware costs
  • problems with revision surgery
  • awkward and time-consuming

INJURIES OF THE CRUCIATE LIGAMENT AND MENISCAL LESIONS

Over the past decades the number of injuries of the capsule ligament apparatus of the knee joint has steadily increased. This is in part due to the intensive leisure time activities of large sections of the population. Ball sports and skiing are amongst such knee-joint-stressing activities. Acute injuries of the knee joint can be reliably diagnosed thanks to clinical diagnostics such as arthroscopy and magnetic resonnance tomography (MRI).
The anterior cruciate ligament is amongst the most frequently injured ligaments of the knee joint. Only in a few cases an isolated injury of the anterior cruciate ligament is found. Rather, in 3 out of 4 acute ACL ruptures, concomitant injuries are diagnosed, out of which 75% are meniscal lesions.

INTERFERENCE SCREW

A simple technical solution for direct fixation of the transplant in the femur and tibia using interference screws generates a stable fixation close to the insertion. The method is established all over the world. The interference screws compress the bone plug or ligament in the tibial or femural drill tunnel, thereby promoting fast bone growth. This allows fast functional aftercare, which again reduces swelling and pain.

ADVANTAGES OF BIORESORBABLE SCREWS

Fixing the transplant with metal screws is a widely used method. Implanted metal screws, however, may have a negative after effect, such as hypersensitivity to metals, stress concentrations on the bone, metal artefacts under radiological follow-up examinations. This pushed the development of bioresorbable interference screws. The Richard Wolf Bioact IF Osteotrans bioresorbable interference screws guarantee the same initial ancoring as metal screws. Over time, however, they dissolve slowly and are replaced by the patient's own bone tissue.

ORTHOPAEDICS / ESWT / TPST

The PiezoSon 100 plus is a small, powerful desktop device for extracorporeal shock wave lithotripsy of all orthopaedic ESWT and TPST indications. The heart of the unit is the compact Wolf Piezo dual-layer shock wave source; this allows a small and ergonomic design without forfeiting any power. The device is controlled from the front panel of the multifunction operator control and display unit. All the device parameters can be set and monitored with fine precision.


With its multiple connector, gel-filled therapy sources of varying designs can be adapted. With the gel-filled waterless therapy sources, the depth of penetration can be varied by using pads of different thicknesses. New acoustically adapted gels now allow the focused shock waves to be adapted without a water-filled coupling membrane. All shock-wave sources of the adaptable therapy sources of the PiezoSon 100 plus have the new dual-layer design. Products

INSTRUMENTS FOR ACCESS

Stable puncture cannulas allow direct passage through all soft tissue structures for the primary access to the joint under X-ray control and the creation of other access portals under endoscopic control. The semi flexible guide wires made of Nitinol (Ø 2 mm) are introduced through the inner lumen of the puncture cannulas into the joint and serve as a blunt guide rail for the introduction of all cannulated instruments.
As the first dilator, the cannulated rod expands the access portal to the joint over the guide wire. The trocar sleeves of the arthroscope can be introduced directly over this rod. The semi dish shape of the guide tube is used as a guide channel for introducing instruments. This can be introduced into the joint either via its cannulated obturator or via a dilator (blue).

DILATION SYSTEM

The user-friendly dilation system consists of three colour-coded "one-step" dilators, each with a different diameter, that are introduced into the hip joint one after the other starting with the smallest diameter using the ergonomic handpiece attachment.

ARTHROLUTION-ARTHROSCOPES

The Arthrolution trocar sleeves were shaped specially for atraumatic use in the joint in conjunction with a PANOVIEW telescope with a 70° angle of view. Telescopes with a 30° or a 45° angle of view can also be used. A maintenance-free click locking mechanism guarantees simple, safe and fast connection of the telescopes in the trocar sleeves.

Cannulated trocars allow direct introduction of the arthroscopes into the joint via the guide wire. A new continuous-irrigation arthroscope was developed to optimise the view in the joint. Balanced continuous irrigation and suction allow the joint to be irrigated quickly and guarantee an excellent view.

Longer arthroscopes are also available for obese patients.
